2024-2028 Routt County Public Health Improvement Plan

Every five years, local public health agencies in Colorado are required to conduct a community health assessment and develop a new public health improvement plan based on the assessment results. This requirement is a result of the Colorado Public Health Act of 2008 and creates a way to address leading public and environmental health issues in the county.
